Historical perspective reveals the fact that the modern medical science is the product of a long evolution from the superstition and ignorance of the Dark ages that follows the Greek culture, through the uncritical acceptance of Authority in the Middle ages, the re-discovery & re-interpretation of Greek medical literature and the Renaissance, to the application to medicine of the newly developed scientific methodology of observation, generalization involving logical processes of deduction & induction, framing of hypotheses and experimental verification which resulted in great discoveries of the 19 th century down to the present time. The philosophy of all the scientific advancement and researches was & has still been that of materialism, i.e. the basic reality of the Universe is matter. Modern medical science is still based on this Materialistic Philosophy. Though the subject matter of medicine is MAN who combines in himself the Mental, Vital and Physical aspect.

Medicine, isthe practiceconcerned with the maintenance of Health and Prevention, Alleviation, or CURE of disease. TheWORLD HEALTH ORGANIZATION at its 1978 International Conference, held in the Soviet Union ---- produced the ALMA-ATA HEALTH DECLARATION which was designed to serve the Governments, as a basis for planning Health Care that would reach people at all levels of society. The declaration reaffirmed that - Health, which is a state of complete physical, mental and social well-being, and not merely the absence of disease or infirmity, is a fundamental human right and that the attainment of the highest possible level of health - is the most important World-wide social goal. In its widest form of Practice of Medicine, that is to say the promotion and care of health, is concerned with this deal.

The discovery of Homeopathy cuts across the general evolutionary trained of modern medical science. Dr. Samuel Hahnemann was his own Columbus in the field of medicine. He proclaims both an EPOC & an ERA; he represents both the discovery and progress.

Dr. Hahnemann steps in to put the following questions to himself and his contemporary profession ------

[1] How does it happen, that in the thirty-five centuries since Aesculapuis lived, this indispensable art of

medicine has made so little progress?

[2] What was the obstacle?

[3 ] Why, what physicians have already done in 1/100 th part of what they might and ought to have done? [ vide. 'Aesculapuis in the balance' by Hahnemann,published in Leipzic in 1805]

Dr. C. F. S. Hahnemann devoted the whole of his life to the solution of those problems and the result was THE DISCOVERY OF Homeopathy. Homeopathy is a simple art of healing, unvarying in its principles, and in its methods of applying them. The principle upon which it is based, if thoroughly understood, will be found to be perfect and unassailable, so that the purity of the application, and they are not disobeyed, without sacrificing the HONEST NAME of Homeopathy.

Hahnemann found the enunciation of the law in the remarkable words " SIMILIA SIMILIBUS CURENTUR " [LET LIKES BE TREATED BY LIKES] in one of the booksattributed to Hippocrates, and he also found its foreshadowing in solitary remarks (which he quotes)in words by half-a-dozen authors (viz. Bouldue, Detharding, Bertholon, Theury, von Stœrk &Stahl); " But --- ", as he says, " ---- no one had taught this manner of cure, no one had put it into practice".

The term ORGANON is etymologically derived from Greek word "ORGANUM", meaning the tool or the instrument, however, the term attained its lasting sense during the middle ages, in reference to a specific polyphonic (many-voiced) instrumental setting, capable of producing, in certain specific styles, GREGORIAN CHANTS. When our revered Master Christian Friedrich Samuel Hahnemann compiled his MAGNUM OPUS work on Homeopathic Philosophy, he wanted to use that particular term to set forth his medical philosophy that would best convey its essence since he was precise in his literary expressions. Hence, He adopted Aristotle term ORGANON, so as to illustrate the importance of the instrument or the tool that each & every physician needs to employ for the purpose of curing and that can be used to achieve the sole MISSION OF HEALING.

Since the very beginning of Homeopathy - Organon has acted as an Oracle, giving us the rules & guiding principles that serves as the BEACON LIGHT, illuminating our path that would lead us to the Highest Goal of human endeavour ----- "to restore the sick to health" (§ 1, Organon of Medicine).

Dr. Jnan Majumdar had rightly commented that, " If Materia Medica is a horse, then Organon is its reins that would guide it to its ultimate destination".

According to Dr. Herbert A. Roberts, " The physician should distinctly understand the conditions, what is curable in diseases in general and in each individual case in particular… He should clearly comprehend what is curative in drugs in general, and in each drug in particular …He should be governed by distinct reasons, in order to insure recovery, by adapting what is curative in medicines, to what he has recognized as undoubtedly morbid in a patient ….. Finally, when the physician knows in each case the obstacles in the way of recovery and how to remove them, he is prepared to act thoroughly, and to the purpose, as a true master of the art of healing ". This is a statement in accord with Homeopathic Principles, and with the Laws of Nature governing balance in all things ----- "The amount necessary to effect any change in nature is the least possible". "Action and reaction are equal and opposite".

The word LESION has been originated from Latin word " LAEDERE" meaning "INJURE". Lesion is any pathological or traumatic discontinuity of tissue or loss of function of a part. Thus, ORGANIC LESION means, there is a pathological or traumatic discontinuity or loss of function of the Organic Structure.

Homeopathy itself is a perfect science - but its truth is only partially known. The truth itself relates to the Divine, the knowledge relates to Man. In our day-to-day practice - some cases, which present with some structural and/or functional discontinuity [or with Organic Lesions] - trouble us more. The physician who applies the SINGLE remedy in potentized form under the Law of Cure - found that, in those cases, there are NO alternate ways to administer medicines on the basis of Partial Similar Symptoms or Palliative mode of treatment; and that ONLY holds out any hope for the patient. When any person suffers from Organic Lesion ---- an irreversible process proceeds, the proper Original function is not possible and the person have sense of discomfort. Re-arrangement of the discontinuity of the structure is NOT POSSIBLE. In this condition Medical Aid is required; the CURE IS AN INCURABLE ONE. "After making the prescription - it is our duty to make the observations. The whole future of the patient depends upon the conclusions that we arrive from the observations". It is known to all, that, if a medicine is acting, it commences immediately to affect changes in the patient; and these changes are shown by signs & symptoms.

There are some latent conditions or latent existing Organic conditions / discontinuity - such cases prevent Improvement beyond a certain stage. In those cases that deranged organ is capable of doing ONLY limited work, will have symptoms; and these symptoms will be ameliorated from time to time with medicines - but the patient is ONLY CURABLE to a certain extent; he cannot go beyond a certain limit. After selecting Partial Similar Medicine and prescribing it in lowest possible potency with appropriate repetition - though the selected medicine acts favourably - but the patient is NOT CURED; rather the patient is PALLIATED.

MANAGEMENT OF ORGANIC LESIONS THROUGH Homeopathy: -

The dynamic plane permeates all levels of the organisms just as the electro-magnetic field permeates matter, and it is the origin of all actions of the body in health & disease. A highly organized system reacts to stress by always producing the best possible response of which it is capable. Therapeutic measures, utilizing the dynamic plane, may act either indirectly through a single level; or directly on the dynamic plane itself. Three modes of therapy are capable of acting directly on the dynamic plane; viz.

• Acupuncture.
• 'Lying on of hands' by a spiritually evolved.
• Homeopathy.

The 'Law of Similars' matches the symptom manifested on the dynamic plane in a patient with the analogous symptom of a therapeutic substance manifested in a healthy individual to establish resonance between patient and remedy. The Law of Similar states "any substance which can produce a disease like symptoms or a condition in a healthy human being, can cure the disease or the condition in a natural - provided that it is similar in manifestation but different in kind". The Law of Similars was a basic contribution of Samuel Hahnemann. He systematized this Law by doing 'PROVING', or systematic recordings of symptoms produced by substances on HEALTHYHUMAN BEINGS. To be complete, a proving must be tested in a full range of DOSES (or POTENCIES), the symptoms recorded must included all three levels of the individual; and symptoms cured in sick patient after administration of the remedy must be included.

The diagram presents the Three Layers of human existence in Three Levels. The Mental Plane is the Central conical envelope. The Emotional Plane is shown as the next Peripheral envelope; and the Physical is the Outer-most sheath. This is to be noted that The TOPS & BOTTOMS of each envelope do not correspond one to another.

In the cases with Pathological or Traumatic discontinuity or Loss of functions of the Organic structures, the case being an INCURABLE one ---- I want to find out the efficacy and applicability of the Homeopathy.

Curable cases are those ----- in which the defense mechanism is still strong enough to be able to respond to well selected prescription. On the other hand, Incurable cases are those ----- in which the defense mechanism has been weakened beyond any hope of being able to respond curatively even to a correct prescription. Here the GOAL can only be "PALLIATION" not CURES. In incurable cases the Goal is to find the REMEDY ------ based solely upon the current image. Homeopathy rests on an accurate knowledge of the patient's whole disturbed functions and are restored to normal by a treatment whose indications are "SIGNIFIED" in a characteristic manner by the reaction proper to the individual patient.

Disease is not a primary fact, it is always a result. This result is invariable prepared by a totality of conditions gradually modifying the human soil and giving rise to disorders peculiar to individual, indefinable sensory & functional disorders which can not be clearly defined byOrthodox Clinical Methods. Yet these disorders constitute so many "SIGNS" found in the Homeopathic Materia Medica, enabling us to determine suitable remedy in each case. Diseases, like individuals, present a definite morphology which ONLY the Homeopathic Physician is in a position to appreciate for he "Understands" all manifestations coming under his observations. For him, pain is not merely a manifestation to be alleviated but a phenomenon peculiar to a certain individual "EXPRESSING SIGNIFICANTLY" both the mode of reaction of the patient & the remedy that must be adapted to him. The patient is a "BEARER OF SIGNS" that must be recognized, observed & interpreted according to THREE FOLD APPROACH - Typological, Clinical & Therapeutical. Each Homeopathic remedy experimentally studied, that is to say, "PROVED"; present certain Subjective & Objective characteristics of a sensory and functional nature which together constitute a clinical picture matched by a symptomatic picture in the patient enabling us to determine a suitable treatment, the ONLY one really adapted to the subject under observation. Thus, Homeopathic treatment is essentially based on strict "INDIVIDUALIZATION".

Dr. von Grauvogl, a German Homœopath, described in his Treatise On Homeopathy, published in November 1865 , the " BIOLOGICAL STATES ---- OXYGENOID, HYDROGENOID and CARBO-NITROGENOID" which bear no relation to the morphology of the human being ----- but which help to understand his bio-chemical functions to which certain Homeopathic remedies can be adapted. Dr. Léon Vannier states that, "A patient can never be treated with the subjective symptoms or the symptoms which are narrated by his attendants. On the basis of these externally manifested symptoms, we can neither isolate the patient nor give tentative psychological treatment, nor should be treat his digestive system with an appropriate regime. We shall have to examine him thoroughly through the methods of clinical investigation. Without investigation and personal experiments, all the accurate facts, data, etc., can not be provided". Homeopathic medicines deal indirectly with the chemistry & structure of the physical body by dealing directly with substance and energies of the next, and more subtle, level. This medicine must be classed as a subjective medicine at this time, in part because it deals with energy that can be strongly perturbed by the mental & emotional activities of the Individuals and in part because there has NOT been any diagnostic equipment to support the Homeopathic Physicians.

REMARKS : -

'Management of Organic Lesions through Homeopathy' was my topic of dissertation of MD (Homœopathy) through the University of Pune under the guidance of (Prof) Dr. P. Santaram Kumta, Pune. For the purpose I have Studied 100 cases in the Kamgar Hospital of the Kakasaheb Mhaske Homeopathic Medical College & Post Graduate Institute, Ahmednagar, Maharastra. Among them, detail of 30 cases were presented in the dissertation.

For the purpose, I had considered some of this Homeopathic-By-ways ----

[1] Pathologic-anatomical relationship which is an aid in selecting the remedy.

[2] Locality or Elective affinity.

[3] Etiology of the disease.

[4] Supplementation therapy.

In these cases when the Symptomatology do not come to a Single remedy - covering the Whole of the Case, the case showing whatever the presenting Totality, had been taken for prescribing the Medicine. Such medicine was Partially Similar. In irreversible organic lesion and incurable diseases, or some vital organ is lacking - PALLIATIVE MODE serves better; but the Remedy must simulate, in so far as possible the disease picture.

It is very difficult to predict definitely the cases which are Incurable; so called pathologically incurable cases has been completely cured by Homeopathic Medicines. Thus, the knowledge of pathology along with Dr. James Tyler Kent's 12 Observations had been carefully considered to decide the curability of the cases. While assessing the cases, the Inclusion and the Exclusion criterion had been strictly followed.

The INCLUSION CRITERION were ----

• Cases with morphological disproportion.
• Cases where there are irreversible organic changes or advanced structural damages.
• Cases with hereditary inheritance.
• Cases where the patient is lacking in any vital organ/function; Congenital and/or Acquired.
• Cases with irreversible Post-Surgical complications.

The EXCLUSION CRITERION were ----

• Pseudo-chronic diseases.
• Idiosyncratic patient.
• Artificial Chronic diseases.
• Cases where patients have long-continued drug-habits.
• Cases presenting with One - Sided Disease & Alternating Diseases.

While assessing the cases, the rules for prescription were ------

• Is there anything in the etiology of the disease which would help me to select a drug?
• Does inspection of the patient and his actions call to mind a remedy?
• Do the complaints of the patient correspond to the characteristic sensations & modalities of any drug we can think of offhand?
• Miasmatic Inheritance will be kept in mind.
• Lower potency and/or Mother Tincture will be employed; as with the Irreversible Organic Changes - medicines in CRUDE FORM is preferable [according to Dr. M.L. Tyler].
• With the Lack of Prominent Symptoms, medicine has been prescribed on the basis of its Organ Affinity.
• Administration of Mother Tincture will depend on its Organ Affinity or Sphere of Action --- whenever the necessity will arise. For the purpose of study the Dose and Repetition of the prescribed Tincture ---- was 10 drops 03 times daily in ½ cup of water (at a stretch) for 07 days - in Adult; and in Child that was 05 drops 02 times daily in above mentioned way (as per WHO Adults are considered above theage of 12 years).
• Single Potentized Medicine (in Centesimal scale) with Partial Similar, following the Homeopathic rule - was administered; Biochemic medicine was given in cases where Supplementation is required ---- 01 tablet once daily for 7 days.
• With regard to the Duration of Action of the Prescribed medicine and the Progress of the Improvement of the case ---- Repetition of the medicine was considered; as our Master Dr. Hahnemann instructed in § 248 of his Organon of Medicine ( 5 th edition.) - "The dose of the same medicine may be repeated several times according to circumstances, but only so long as until either recovery ensues, or the same remedy ceases to do good and rest ofthe disease, presenting a different group of symptoms, demands a different Homeopathicremedy".
• Follow-up of the case will depend upon the Acuteness & Chronicity of the Case. In Chronic cases the follow-up done initially at a fortnight for 3 months; later on, once in a month. In between, if the patient develops acute ailing, will called for the observation for 24 hours.
• The assessment of the observations done on the basis of case-assessment. The assessment done on the basis of changes in Functional result and Investigation Reports.

In the course of my study the detail observation of the cases between January 2003 to November 2004 in the Kamgar Hospital OPD of the Kakasaheb Mhaske Institute. I had followed the strict rules as led down by our Master Dr. C. F. Samuel Hahnemann. All the cases were treated with Homeopathic remedies according to Symptom-Similarity and the cases were observed for at least a period of SIX MONTHS.

OBSERVATION AND ANALYSIS : -

All the age groups and both sexes have been considered. Each case was observed for a period of 6 months. I had observed 24 Adults and 06 Children patients. Among the adults, 15 were Male; 09 Female. There were 06 Male Child and Female Child - None. The Miasmatic Predominance were ----

• Psora - in 06 cases.
• Sycosis - in 01 case.
• Syphilis - in 01 case.
• Psora & Sycosis - in 19 cases.
• Psora & Syphilis - in 01 case.
• Psora , Syphilis & Sycosis - in 02 cases.

For the purpose of study I followed the Homeopathic Principle; in addition to that, it may be supplemented by -----

• Pathologic - anatomical relationship which in an aid in selecting the medicine.
• Locality or Elective affinity.
• Etiology of disease.
• Supplementation Therapy.

While studying the 30 cases, the presenting symptoms - due to the Lack of Characteristics - Partially Similar medicine was chosen. This Partially Similar Medicine served as Palliative. In some cases where, there was Deficiency Disorders - Congenital or Acquired - Supplementation Therapy has been employed. During the study Biochemic Potentized Medicine was given in Supplementation Therapy ---- and in others, Centesimal Potency was administered.

ABSTRACT : -

The topic of Dissertation, " Study of Scope of Homeopathy in the Management of Organic Lesions " , was a new and different experimental study. Lesion is any pathological or traumatic discontinuity of tissue or loss of function of a part [ vide. DORLAND'S POCKET MEDICAL DICTIONARY]. Thus, ORGANIC LESION means, there is a pathological or traumatic discontinuity or loss of function of the Organic Structure.

For the purpose I had gone through 100 different cases and 30 cases were studied & observed in detail ---- which were presented with Structural and/or Functional Discontinuity. In the course of the experiments, treatment was conducted on the Homeopathic Principles.

The cases were lacking in the Characteristic Symptoms - thus Exact Similar Medicine was NOT indicated. On the availability of the Symptomatology - the most indicated were only partially similar and out of these, the best & the most nearest were selected. Experimented cases were administered in suitable low potency. Due to Structural and/or Functional Discontinuity - it was observed that NONE was cured, but has shown encouraging result. In the observed cases - Irreversible Organic Changes / Structural Damages were more [ 53.33% ]; among them overall 81.25% cases improved. And the Miasmatic Predominance of Psora - Sycosis were maximum [ 63.33% ]; among these 84.21% cases showed overall Improvement.

Among the improved cases:-

• 36.6667% cases showed 25% Improvement.
• 43.3333% cases showed 50% Improvement.
• NO case showed either 75% or 100% Improvement.

CONCLUSION: -

The Findings of the presented case - study were ----

• The cases with Irreversible Organic Changes/Advanced Structural Damages were the more and they showed more improvement. The cases studied were 53.33% - among them overall 81.25% cases improved.
• The studied cases with Lacking in Vital Organ/Function - Congenital or Acquired were 30% and among them 66.66% improved.
• As the cases with Morphological Disproportion, Hereditary Inheritance & Irreversible Post - Surgical Complications were studied in a very small scale [ viz. 06.67%, 03.33% & 06.67% respectively] ---- the response of those cases cannot be considered as per Statistical rules.
• Among those presented 30 cases Psora - Sycosis were prevalent in 63.33% cases and 84.21% improved.
• In some cases where Syphilis , Psora - Syphilis & Psora - Sycosis - Syphilis were prevalent - were found to be in a very small number of cases [ viz. 03.33%, 06.66% & 09.99% respectively] and their response cannot be considered according to the Statistical rules.

Among the studied cases, I had observed overall Improvement in 80% cases and 20% cases had not improved. Among the Improved cases --------

• 25% Improvement found in 11 cases.
• 50% Improvement found in 13 cases.

For the purpose of the study; the considered Homeopathic method required were ----

• Pathologic - Anatomical Relationship.
• Locality or Elective affinity.
• Etiology of Disease.
• Supplementation Therapy.

In these cases, the presenting symptoms were indicating Partially Similar Medicine - due to the lack of characteristic symptoms - which is required to choose the most Similar Remedy. It has been found that, this Partially Similar Remedy served as Palliative. The method followed, showed Encouraging Results for the reduction of the suffering of the patient with Organic Lesions under the Homeopathic Management.

Thus, I can conclude that in spite of the limitations; we can manage the patients, in whom the Organic Lesion has taken place. We can help the livelihood of the patient in an APPARENTLY HEALTHY CONDITION.
